• LUNG GUIDELINE:

  • GA/GA with epidural

  • Dosing by age: Dilaudid bolus via epidural

  • < 60 years = 0.6mg

  • > 60 years = 0.4mg

  • > 70 years = 0.2mg

  • At beginning of case initiate

  • Optional:

    • Ofirmev 1000mg

    • Robaxin 1000mg (NOTE: this can be split 500mg at beginning/500mg at the end)

    • Ketamine: 50mg IV over 2-3 hours

    • Precedex: 0.4mcg/kg/min

  • CVP (if DL in place) <10

  • Overall fluids <1000ml
